Sarnak most frequently takes a form that is a cross between a troll and an orc, but with a terrible difference: he has four arms and three eyes; black skin and ivory fangs. The third eye is centered in his forehead; it is said that it sees only in the spirit world. Some say that Sarnak is of the progeny of Cthon.
In his upper left hand, Sarnak weilds a torch; in the lower left, a scimitar. His upper right hand is splothed with red and carries plague; his lower right hand bears the Jug of Misfortune, from which many bad things flow. Wheerever he passes, structures collapse, rocks crumble, and plants rot. It is said that when Sarnak smiles, nations fall.
